I've played the game now 4 or 5 times, survived the war once. Along the way I have figured out a number of things (despite the lack of a manual or many useful game guides/walkthroughs).
One thing that puzzles me still, is the hospital and how you are treated there by the armed people inside. Once I did rob the hospital and get called out as a thief and after that the guards would shoot at me if I tried to go back. So on other games I left the hospital alone, I did not touch a thing, because I wanted to retain the ability to send a really sick or wounded survivor to the hospital.
In my most recent game I did exactly that, I sent Roman to the hospital because he got badly sick. He walked past the guards, no problem, to the second floor. I clicked on a doctor. Roman seemed to be swallowing some pills when a guard ran to the floor and shot-killed him (also wounding the adjacent doctor). After that my group was not welcome at the hospital.
Every other game I have played has a similar outcome eventually. At some point in the game, the hospital stops being a friendly place and an attempt to go there always has my scavenger running into a guy at the door yelling "I'll kill you!" while holding an assault rifle, forcing me to turn and run and sometimes getting shot if I hesitate too long.
So either there is a game glitch in the mechanism that tells the hospital guards that you are a bandit. Or something big happens at the hospital. I mean it transitions from being a friendly "we'll help you" place, to a place that is taken over by bandits and adopts a siege mentality. If its the latter, that seems to be a reoccurring transition in the game as time progresses.
So...which do you think it is? Or am I missing something entirely?
